Our Dental Savings Plan is designed to provide affordable dental coverage and greater access to quality dental care! There is NO MEMBERSHIP CARD NEEDED! Your plan renews the first of each calendar year.

Program Guidelines

• Patient’s portion is due on the day of service
• No refunds or premiums will be issued at any time if participant decides not to utilize dental plan
• Cannot be used in conjunction with another dental plan
• Cannot be used for referrals or specialists
